An Online Connection Between You and Your Provider

We want to improve your healthcare and make your office visits easier and more convenient.  That is why we have set up a free, secure website, called a patient portal, which lets you get health information and connect with us right from your computer.

What is a Patient Portal?

With a username and secure password, your patient portal lets you:

• Get health information, anytime, day or night.

• Update your address, phone number and health history.

• Check lab results.

• Ask for medication refills, lab tests and referrals.

• Get reminders about appointments.

• See your bill.

• Send and receive routine messages.

Your patient portal is not for emergencies.

How to Get Started

• Ask our front desk staff to be web enabled and obtain user name and password.

• Go to:  https://mycw3.eclinicalweb.com/uaod/jsp/login.jsp

• Enter your user name and secure password to log in.  The first time you log in, please read and accept the terms of use of the portal.  Then, update your personal information as needed.

• Be sure to keep your log-in information in a safe place so only you and your provider can see your health information.
